Paving the Way for Progress: The Agency for Healthcare Research and Quality Patient Safety and Medical Liability Demonstration Initiative
Although the United States provides some of the best health care in the world, unsafe health care processes continue to harm patients and families every year. According to the most recent final data, nearly 4 million adverse events occurred in the U.S. hospitals in 2013. In 2014, AHRQ's preliminary estimates show that there are about 170,000 people who died as a result of an adverse event or medical error. Based on the best available data, we estimate that about 44 percent of adverse events are preventable—this is approximately 75,000 deaths for 2014 (AHRQ, 2015).
